The standard annual premiums this bond depend on bond amount:

	Bond amounts up to $2,000 = $10 per $1000 of bond amount
	$2,000-$50,000 = $20 plus $5 per $1000 of additional bond amount
	$50,000-$200,000 = $260 plus $4 per $100 of additional bond amount
	$200,000-$500,000 = $860 plus $2.50 per $1000 of additional bond amount
	$500,000-$1,000,000 = $1,610 plus $1 per $1000 of additional bond amount
	$1,000,000 Bond = $2,110 plus $.50 per $1000 of additional bond amount
