The standard annual premiums this bond depend on bond amount:

	Bond amounts up to $5,000 = $10 per $1000 of bond amount
	$5,000-$200,000 = $50 plus $6 per $1000 of additional bond amount
	$200,000-$500,000 = $1,220 plus $4 per $1000 of additional bond amount
	$500,000 = $2,420 plus $2 per $1000 of additional bond amount
